The covers here only reflect the Region 1 release, not the original Region 2 release which has more covers. Below each lot of DVDs I put up the episode titles for future reference. Also, as a side note, the first DVD came out with a box and a regular edition (both listed below), however, fans started complaining about the cheap quality of the box and ADV later favored the fans by providing us with a much more sturdy and elegant box with the movie. I took a few pictures of my copy, check it out. There was also a "thinpack" boxset released, but I don't own a copy of it so I couldn't take any proper pictures.
